What should I expect as a NEW patient?

Welcome to Portrait Dental!

As a new patient, we will ask that you fill out a patient history and medical form.  For your convenience, these forms can be found here or you can fill them out at our office however we ask that you arrive 15 minutes prior to your appointment to complete them.   Please also bring any insurance information and/or cards that you will want to use for your first and future visits.

It is recommended that you book a complete oral examination which includes a complete thorough cleaning and x-rays so the dentist can evaluate your current dental health and determine if there is any pressing treatment needed.  If you have any concerns with your oral health, treatment goals, or questions, it would be helpful to bring a list with you.

We’re happy to answer any questions and welcome you to our practice!

What is a complete oral exam?

A complete oral exam is a comprehensive diagnostic exam so we can evaluate any oral health conditions or concerns. During this exam, the hygienist and dentist will clean your teeth, take x rays and check for cavities and gum disease.

Do you do direct billing to my insurance company?

Yes, we do! Depending on your insurance plan, we can bill your insurance company directly and immediately after your appointment.
